2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

I used the same donor and have the most wonderful beautiful healthy and sweet little boys in the world!!!! I would love to share pics, hes a real ladies man already with his crazy long eyelashes and dark brown eyes, just like our donor!!
