2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

Hello Ladies,
I have 1 vial available for donor 2799, prep type ICI. This is available thru December only. This donor is no longer active. It is located at my Reproductive Endocrinologist's office in NJ. I was told the way this works is fairfax would do a transfer between us both (I believe there's a fee for that, maybe $100, but not sure exactly). There would also be a shipping fee. I am selling for what I paid $665 + shipping, I already paid shipping to the office
