Clinic Name
Donor number
Child Date of Birth

Contest Rules:
Contest is open to legal residents of the U.S. who are 18 years of age or older. To enter, go to our Facebook page, "like" us and add a caption to one or more of the contest photos. Visitors to our Facebook page will be able to "like” their favorite captions, and we will award $100 gift cards to the 10 people who post the most-liked captions (and who have "liked" our Facebook page). Gift cards may be used for any product or service we offer, including donor semen vials, shipping, storage and donor information. Use your gift card by January 30, 2015 and get half-off our regular shipping charges. Contest begins at 12:01 a.m. Eastern standard time on December 4, 2014, and ends at 11:59 p.m. Eastern standard time on December 18, 2014. Winners will be contacted through Facebook no later than December 19, 2014. Limit one gift card per household. We will remove and disqualify captions that are offensive or profane, in our sole discretion. Gift cards will expire on December 31, 2016. A complete set of contest rules will be available on our Facebook page during the contest.
