Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

Seeking A DC Area Individual or Couple Willing to Share Your Story with Japanese TV

A Japanese television network wants to interview an individual or couple about their experience using Fairfax Cryobank to build a family. The network is also planning to visit the Cryobank and to interview Dr. Michelle Ottey for the story, which will air in Japan and be available via the internet In the US.
The producers understand that some individuals or couples may not wish to have their entire names used in the story and are willing to use only a first name.
Interviews will take place in the month of December 2014.
