2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

If anyone is interested, I have 4 vials of iui for donor 4267. They are currently being stored at OU Reproductive Medicine in Oklahoma City, OK. However, they are able to be shipped out when you need. We have a one year old boy that was conceived on the second try with this donor and I am 15 weeks pregnant with baby number two that was conceived on the first try.
