2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth
*If you are looking to start a private group for a PRS donor please email forum@fairfaxcryobank.com with the above information and that you are looking for a PRS group*

Hi everyone, my wife and I are trying to start our family. We had genetic testing done and have found out that I am a carrier for SMA. We had already purchased vials from 4715 and now possibly won't be able to use them due to the risk of having a child affected with SMA. We are writing to say that we have 2 IUI vials available and are willing to sell them at a discounted rate. They have been safely kept at the doctors office the entire time, obviously. :-) Please reach out with any questions or if you are interested.
