2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

Any connections to this donor? Is he active still?
My wife and me are trying to going through this infertility issues now. Need to find a donor that is like me which is hard since I'm multi-racial of African American and Irish decent. Anyway, if any one has anything on 5043, it would be appreciated.
