2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

In the beginning stages of my IUI journey and have #4334 in my top 3.
Would love to hear about other journeys as well. To say I'm a bundle of nerves and fears is the understatement of the century.

But my wife and I used donor 4988 via IUI and now have a 10 month old! Was wondering how all your journey's worked out!!
