2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth
*If you are looking to start a private group for a PRS donor please email forum@fairfaxcryobank.com with the above information and that you are looking for a PRS group*

I am currently pregnant with 4991 at 30 weeks and we have plenty extra for siblings. Hope you had luck and that is why you are selling the extras.

Unfortunately I was only able to attempt once last year with Donor 4991, and I had to stop due to another medical issue. I've held in storage since then but decided to sell as I won't be able to continue this path right now
