2 vials with abormalities and NO motility

I woke up from my retrieval to find out the 2 vials we had had severe abnormalities and no mobility. Has this ever happened to anyone or anyone you know? Looking for answers and can't seem to get any. We know it is not that specific donor because we have used him for four IUI's with all good counts.
