2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth
*If you are looking to start a private group for a PRS donor please email forum@fairfaxcryobank.com with the above information and that you are looking for a PRS group*

Hello all, I have a beautiful, healthy 2 year old daughter from an IUI with donor 4386. I am seeking anyone else who may have children using this donor's sperm to connect about any health issues and other general connections.

I am so thankful you have put this post out there.
I am just starting this process for myself but have decided to use donor 4386’s sperm. Although I do not yet have any children, I hope to have conceive shortly.
If you are open to it, we can touch base or discuss further once this happens.
