2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth
*If you are looking to start a private group for a PRS donor please email forum@fairfaxcryobank.com with the above information and that you are looking for a PRS group*

Hey everyone!
A few years ago I was in this spot, on the journey to motherhood, which was complicated and long. Lately, the idea of creating an application that would help us in tracking and dealing with fertility treatments (injections, u/s, lab results etc), came up. I recruited a few developers as well as graphic designers and we all got together voluntarily in order to create a free and non-profit application for women within this journey. I would appreciate it if you could take 3 minutes of your time to answer 9 short questions that would help us to characterize the application and to match it to your needs and wishes. Thank you!
