2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth
*If you are looking to start a private group for a PRS donor please email forum@fairfaxcryobank.com with the above information and that you are looking for a PRS group*

To locate a vial left from donor 1914...

Hello I had bought from you 2 vials from donor #1914, and on the first one I got pregnant...I was inseminated in the KU hospital in Kansas city, where the clinic
of my doctor was located... so my question is how can I find out about that vial left? if is in the hospital or in the clinic... cause I found out that the clinic is no
longer in the hospital... I will appreciate if you can help me with this matter... thanks in advance...
