Trying for the 2nd time and makin some changes!

edited 2011 10 in Same Sex Couples
My wife and I are trying for the second time in August and are looking for as many answers to this as possible. How many hours after +OPK should we start to insem?
We plan to purchase 3-4 ICI vials and try again at home. In June we tried for the first time. We went gung ho and bought 6 vials and insem for 4 days straight about 24 hours apart late in the evening starting the day before we knew there would be a +OPK. Our OB/GYN had said to start early as not to miss it. We were not using the smileys tho and were using strips that were most likely less accurate (+ would show for 2-3 days with usually 1-2 days of that being very dark-confusing a bit...lol) :( this time we would like to be more specific.
We have read like crazy and talked to many people and gotten advise a hundred different ways and are just trying to take count for what people who have had BFP and BFN to see where the common denominator is time wise anyway.
Since we have read that frozen thawed sperm only lives approx 24 hrs with the first 12 being the best and the egg only lives approx 24 hrs with the first 12 being the best we're thinking that 24 hrs apart was maybe too long? Maybe we were too soon or too late? We will be using the infamous smileys this time and checking every 4 hours fo rthe positive starting about 2 days before we would normally expect the +OPK. We are just unsure of when after getting the smiley should we start insem and how many hrs apart thereafter?
Any advise at all is so greatly appreciated!
SIncerely, Thank You.

Kathryn & Stephanie

I don't have any words of wisdom for you, but I thought I'd respond to let you know that we're in the same boat that you are! Going for attempt #2 this month, and trying to figure out all of the same things. Reading, charting, rereading, planning ... it's enough to drive you crazy.

I put together a timeline today, because I am a dork. Using all the information that you stated above (and knowing that I'm doing in-office, unmedicated IUI x2 per cycle, and I pretty-much need to dictate the timing), I think that I'll be trying to insem at roughly 22 and 40 hours post +OPK (so ... 18 hours apart - my FP Doc suggested 12). I literally put together blocks of time and laid them on top of each other to see the overlap and that's what I came up with. I'm a pretty visual person, so this really helped me.

Good luck to you guys, I'll be hoping for BFP's for both of us!

Im pretty sure those that do it after a smile its 12-36 hours before on actually ovulates. That said if your waisting that much money a month in vials why not cut it down to one vial and have the doc do it? You really are waisting alot of money and time then necessary
  • 2MC: Thank you for the encouragement!! We are still in the air as to hours, debating between right at the +OPK and then either 12, 14 or 16 hrs after for the last 2 vials. We will see. We are debating in office IUI also, but really wanted to try a few times at home first. Here's hoping we both have good news very soon!!! :D

Honestly if you go at any of those times you probably wont have luck. From what I have been told once you see a smile it just a sign you will ovulate in the next 24-48 hours. Donor sperm doesnt live as long as normal fresh sperm. If you go at those times you are wasting it. if you insiste on doing more then one, do the first at 12-24 hours after the smile then again at 36 hours(best time).

I have to say I agree with Bluesky10 that it seems you may be jumping the gun a little on the timing. I wouldn't bother inseminating any earlier than 18 hours post +OPK. Then I would try again 12 -18 hours after that (some suggest 24, but if those little guys only live for 12 hours then that leaves 12 hours of dead space in the middle there).

Again, good luck! With juggling the clinic hours and my body's insistance on surging overnight, I ended up with the (not ideal) schedule of insem at 24 hours (or more) post +OPK, and then again at 34 hours +OPK this month. Both today.

Hope things go well for both of us this time. :D

  • I guess I should have been more clear..lol I meant timing wise at 12-16 hrs post + for second vial and again 12-16 hours after that for 3rd vial. So all in all covering up to 32 hours post +. But maybe we will push it to 18 hours for that 2nd vial and then only 12 later on the 3rd. Good luck 2MC!! And thank you both for all your advice, sorry for the confusion.

Really you are doing the ICI way way too early. The positive OPK is saying you will ovulate in 24-48 hours. You should be doing your first ICI at 24 hours post positive, and then another after another 24 hours. The way you are doing it, you are wasting two vials before you even ovulate, and by the time you do ovulate those 2 vials of sperm are already dead.

I saved money and went to a doc right away. I ovulated at 33 hours and had an IUI at 36 hours. Most docs will tell you if you time the IUI/ICI right more than one vial is a waste.

I suggest doing more research before you do another round to save yourself time, money and heartache. Have you done any testing at the doc? The reason I ask is that its important to make sure your body is even ovulating. Just because you get a positive OPK doesnt mean ovulation is happening.
  • I agree with bluesky. We have tried ICI at home, non-monitored/ no drugs IUI and now fully monitored. Going to a clinic that specializes in infertility is worth it and not even that expensive. I would also get the blood tests done first to make sure you " can" conceive. I found out trying 6 times naturally that my progesterone was low. Wish I would have known that right away. If you want to chat further, send me a message.
  • we have had blood tests done and got the green light from the doc to start trying. All my levels are in good range and my doctor said at this point she sees no reason wny I wont get pregnant. We have only tried one time. At this time I'm not really worried about it, was just looking for thoughts on timing.
  • You could also try using other fertility signs to help. If you know your body and fertility signs well enough it can aid in figuring out timing. I wasn't sure if I missed my OPK or not due to the confusing non smile strips, but I checked cervial mucus, which indicated fertility so I went into the doc that morning for the insem and we ended up pregnant.
  • Got any updates???

I thing trying at home is just wonderful. I am considering that for the next go round (my partner will be carrying our next child, but if we go for # three, I'll be up at the mound again). I can imagine it makes it much more intimate.

Here is my advice to you (if you are not already pregnant, that is!)... Use the cheapie OPK's for the few days leading up to the positive OPK. When you are unsure of the +/-, use the smiley OPK's. If you are checking every four hours, you should get pretty accurate timing. If you are using 4 vials, I would insem at 12, 18, 24, and 36 hours POST +OPK. Don't insem before the +. If you are using three vials, I would do 12, 24, and 36. If you are using two vials, I would try at 18 and 36 hours.

This is just my two-cents. I got preggo on the first try (granted we did IUI unmedicated at the clinic) using two vials and insemed at 18 and 36 hours. Every body is different, but I went with the most common time-table suggested by my doc.

Good luck to you and your wife!!!
