Follow these steps to join a private donor group:
1) Press "Join" at the right of the group
2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth

Check Updated Medical History

How do I check the updated medical history of Donor 1073 (donated between 1985 - 1987) if I don't have anything other than the donor number? The doctor who ordered the vials was Dr. Frederick (Gary) Asmussen, Dubuque, IA - Mercy Health Center (I think that was the name of the clinic).

    To check on the status of a donor you will need to have ONE of the following:
    •your customer ID number or
    •physician account number or
    •order number from your invoice or packing slip
    And your Donor ID Number.

    If you are having problems or do not have this information, please call Client Services at 800-466-2796.

    If you are an international client, or units were ordered for you by your doctor's office, you will not have a customer number. You can contact Client Services by email and provide them your name, name of the group that ordered your donor sperm for you and the donor number. They will email you your donor's medical status.

    CanAm Cryoservices clients can contact Client Services for the CLI account number to use or contact CanAm directly.
