Trouble contacting CLI

I hope this is the appropriate forum to get some answers. If not, I apologize. I contacted CLI via email to obtain some information about my healthcare practitioner to establish an account, and they told me that I could call and provide them with a fax number for my licensed healthcare practitioner and they'd send the appropriate forms for her to establish an account. I sent the fax number to CLI through email (a disability prevents me from calling) to have the appropriate forms faxed to my healthcare practitioner so she can set up an account, but I haven't heard back from anyone and she hasn't received any faxes from them. I'm not really sure what to do or where to go to get the forms faxed to her so she can set up an account so that I can place an order. (I asked my healthcare practitioner if she would call, but she asked me to take care of it for her (i.e, get her the appropriate documents.) Please help.

    I apologize in the delay of getting those forums to the appropriate health care provider. I know getting these taken care of can be time sensitive.
