What gets listed in medical history?

How do you determine what is "bad enough" to list in the medical history? There was a donor that was listed that had a good medical history but after listening to his audio he mentioned having surgery for lazy eye. The instantly rule him out for us, however this wasnt listed in his medical history....

Donor 4122 update received May 2014: Donor reports that he had corrective surgery for lazy eye (amblyopia) at age five, with no recurrence. There are no other instances of the condition in any primary or secondary family members.
