Question about birth certificate...?

As a single Mother-to-be, I have a bit of a unique question and I'm not sure if anyone knows the answer...

I was married young in 2002 and later divorced in 2006... At the time, I was working in an industry where your name is your business, so I chose to keep my married last name - and have continued with that identity for nearly 10 years, now. At my parents' request (which I wholeheartedly agree with), I have decided to use my maiden/family name as the baby's last name-- which prompted a discussion last night about my own last name. I had thought that I could just keep my name while still giving the baby a different last name but my parents said that wouldn't be allowed, unless the child was being given the "father's" name, which would also need to be on the birth certificate- and we all know that isn't an option here.

Basically, what I wanted was (for example): ME- Jaime "Smith" & CHILD- Baby "Johnson" ... But what I'm being told is that I have to revert back to: ME- Jaime "Johnson" & CHILD- Baby "Johnson," in order for the baby to have my maiden/family name.

Is there anyone out there who knows the correct protocol- or has been in a similar situation, or knows someone who has?

    When I worked at a hospital, the protocol is to name all babies, "Babyboy/babygirl" (mom's last name)....so even if parents are married or unmarried and father's name is Smith and mother's name is Johnson....baby would be known as "Babyboy Johnson". Believe me, fathers often got upset about this one....

    However, this is only for medical records...

    But later the birth certificate can be any last name (I think)....double check on that one.

    Pretty sure it can be any name. For example my daughters is like this "smith-Johnson"

    Thanks, ladies! And bluesky, it definitely makes me feel a lot better knowing that your baby was able to have a hyphenated name which was a bit different than your own :)
