Name (under which the vial was purchased)

Clinic Name
Donor number
Child Date of Birth

As the genetic counselor at Fairfax Cryobank for the last 8 years I know firsthand about the toll infertility takes on our clients. About 1 in 3 who use donor sperm do so because they are experiencing infertility with their male partner. It can be a rough journey with many ups and downs.
I volunteered to lead our team because like many of you, I too have faced this challenge. After many infertility treatments and 5 miscarriages, I was finally able to conceive at age 42. My son is now 8 years old and the light of my life! If you live near DC or just want to support this important cause, please visit our team page and invite your friends too!
