2) Once prompted to confirm your request please list this information so we can verify your information:
Name (under which the vial was purchased)
Email
Clinic Name
Donor number
Child Date of Birth
*If you are looking to start a private group for a PRS donor please email forum@fairfaxcryobank.com with the above information and that you are looking for a PRS group*

Hi all -
We are selling 4 IUI vials of #2984 along with the child and adult photos, full medical and personal history, full audio interview, and the silhouette and Keirsey donor report.

He is currently inactive and being sold for siblings donors only through Fairfax Cryobank.
We paid ~$750 for each along with purchasing the reports and photos but will sell all four specimens along with the reports/photos for $2500.
