Passports for Children?

Hello everyone!

Well, this is a new one for me. We are going away in November and I need to apply for my daughter's passport. The material states that "Both parents must appear in person with children under age 16. If this is not possible, the non-appearing parent may complete a Consent Form, sign it in the presence of a notary, have it notarized, and bring a photocopy of the front and back of the ID shown to the notary."

Do you ladies have any experience with this? Her birth certificate lists only me as her one parent.

    I got my daughter's passport when she was 5 weeks old. Her birth certificate only lists me as the mother. No issues whatsoever. They don't need any additional forms if you are the only one on the birth certificate. The hardest part was getting the photo with her eyes open, LOL!

    I also laugh that her passport says she is 21 inches tall and 8 lbs when she is already 13lbs and 25inches

    Enjoy. However, I would recommend traveling with her passport AND birth certificate so no officials question if you are abducting her.
